How it is madeA radial disc with a flatter recessed well inside it and four holes, each with a pale lower lip, because a hole in a shiny material is read by its lip rather than by being dark. The thread is drawn before the holes so it looks threaded through them, and both tails run off toward where they would enter the cloth.
